PLEASE NOTE: Our cancellation guidelines have been updated to require a minimum of 48 hours notice. Appts for 11/24-12/31 may require a deposit & will have a $250 cancellation fee

Pre-Appointment Instructions

  1. Immunizations and Dental Work: Avoid filler appointments if you’ve had immunizations or dental work in the last 14 days or have them scheduled within 14 days after your appointment. Contact us to reschedule.

  2. Illness: If you are ill, please reschedule your appointment.

  3. Blood Thinners: Stop taking blood thinners (e.g., Advil, Ibuprofen, Vitamin E, Fish Oil, etc.) 7-10 days before treatment. Consult your doctor before stopping prescribed medications.

  4. Bruising and Swelling: Bruising and swelling are possible after injectables, even if you’ve never experienced them before. Bruising can last up to 2 weeks, swelling up to 4 weeks.

  5. Alcohol: Avoid alcohol for 24 hours before your appointment.

  6. Cold Sores: If you have cold sores or open sores in the treatment area, please reschedule.

  7. Eat Before Appointment: Have a meal or snack before your appointment.

  8. Intake Form: Your intake form must be completed prior to your appointment. If it’s incomplete by the day of your appointment, we may cancel your appointment.

Cancellation Policy: We require at minimum 48 hours notice for any cancellations or changes to your appointment. Patients who provide less than 48 hours notice, or miss their appointment, will forfeit their deposit; if no deposit was required for the scheduled appointment, the card on file will be charged a flat fee of $100. Repeat late-notice cancellations may be subject to additional deposits to book.

At Lively Aesthetics, we look forward to your visit & are committed to providing comfort and expertise. If you anticipate being late, please inform us as soon as possible. We cannot guarantee services if you arrive after your scheduled start time.

Amy Cook RN, CANS

Amy is an Aesthetic RN with over 6 years of experience in the aesthetics industry, and is also CANS Certified (Certified Aesthetic Nurse Specialist). She began her career as an esthetician and developed a passion for medical aesthetics. Her entire career has been devoted to working in the cosmetics industry. Amy’s background and experience in skin care and facial plastic surgery gives her a thorough understanding of facial aesthetics. She loves educating patients on treatment options and helping them create plans to achieve full facial rejuvenation. Her artistic eye combined with her meticulous attention to detail make her an ideal injector.

Jennifer Lieb APRN-FPA, CANS, CPSN

Jennifer Lieb is a full practice authority & board certified Nurse Practitioner. She graduated from the University of Illinois College of Nursing with honors and her Bachelor’s degree in 2010 & from Illinois State University with honors and her Master’s degree in Nursing in 2015.

She holds her board certification through the American Nurses Credentialing Center (ANCC), as well as the Certified Aesthetic Nurse Specialist (CANS) certification from the Plastic Surgical Nursing Certification Board.

Jennifer immediately specialized into Plastic Surgery upon completion of her nurse practitioner program and enjoyed working for a decade in reconstructive and cosmetic surgery. She developed the nonsurgical aesthetic medicine program in her plastic surgery role and realized her passion laid in nonsurgical aesthetic medicine.

She established Lively Aesthetics in 2022 to provide highly specialized care focusing on aesthetic medicine and treatment options that allow women and men to age gracefully.

She also proudly serves as an aesthetic injector trainer for Allergan Medical Institute and Revance Aesthetics, training injectors across the midwest on injecting techniques and products.

Natalie Sparks

Natalie Is an aesthetic nurse injector with a background in emergency and surgical nursing. She earned her Bachelor of Science in Nursing from Southern Illinois University Carbondale, graduating Summa Cum Laude in 2023.

After working as a bedside nurse for two years, Natalie decided it was time for a change. She found her passion for aesthetics and took part in a twelve-month apprenticeship at Aesthetic Mentor in Chicago. There, she received advanced education and hands-on training in Botox, dermal filler, and regenerative aesthetic treatments. She is excited to bring what she has learned there back to patients in Central Illinois.

Natalie became a nurse to help people, and aesthetics gives her the opportunity to do just that in a deeply personal way. She is committed to creating natural, undetectable results through individualized treatment plans tailored to each patient’s needs. Above all, she prioritizes patient trust and safety in every aspect of care.

When Natalie isn’t injecting at Lively, she enjoys spending time with her husband, goldendoodle, and two cats as well as squeezing in a Pilates class when she can.

Maddi Van Pelt LE, MA

Maddi is a Licensed Esthetician and Medical Assistant who brings her professional skillset and passion to Lively Aesthetics. She found her love for skincare at a young age and sought to complete her professional education in the field. She is a practiced graduate of the Midwest School of Cosmetology specializing in a variety of skincare services such as facials, chemical peels, and advanced facial massaging. Her passion for esthetics keeps her well-informed with the industry’s latest and greatest technologies and techniques.
